CSI Financial Services, LLC (“ClearBalance,” “we,” or “us”) respects the privacy of our customers and are committed to protecting the information we collect from you. This document outlines ClearBalance’s online privacy practices and applies to anyone who visits www.MyClearBalance.com (the “Site”). It explains how we may collect information from you online when you visit the Site and how we may protect, share, and use that information. It does not apply to information collected from other sources, including off-line information collection. When we collect financial information from you for applying for or participating in the ClearBalance patient financing program, we will provide you with a financial privacy notice that describes the privacy practices of participating lenders, including personal information that we collect for participating lenders.

What We Collect and How We Collect It

We collect personal information from you or about you when you provide this information to us directly. For example, we may obtain personal information when you request information regarding products or services from us, register on our website, respond to surveys, or otherwise interact with us. We collect information necessary for you to set up your online account via the Site and enroll in the ClearBalance Program, including name, address, billing address, telephone number(s), email address Social Security number, date of birth and financial account number(s). You can make credit card and debit card payments through the Site. The credit card information you submit in connection with making a payment is processed by our third-party services provider that processes credit card and debit card payments on our behalf.

If you enroll in the ClearBalance program through the Site we may also collect personal information, including your name, contact information, Social Security number, account information, and health information, from healthcare institutions and other third-party service providers both at the time that you enroll in ClearBalance and on an ongoing basis during your use of the ClearBalance Program. Your use of ClearBalance program may be subject to additional policies and terms of your healthcare institution.

How We Use and Share Your Information

We may use and share information collected to:

  • Respond to your inquiries and fulfill your requests;
  • Enroll you in the ClearBalance program, confirm your identity, and allow you to manage your ClearBalance account;
  • Provide and facilitate the provision of the ClearBalance program;
  • Send you important information regarding your ClearBalance account and the Site, changes to terms, conditions, and policies and/or other administrative information;
  • Send you marketing communications that we believe may be of interest to you;
  • Personalize your experience on the Site by presenting content, ads or offers tailored to you;
  • Allow you to participate in surveys, contests and similar promotions and to administer these activities; for business purposes, including data analysis, audits, developing and improving products and services, enhancing the Site, identifying usage trends and determining the effectiveness of promotional campaigns;
  • Operate, evaluate, and improve our business and the products and services we offer;
  • Analyze and enhance our marketing communications and strategies; and
  • Analyze statistics regarding use of the Site and transactions conducted using the Site.

The information you provide to us, including but not limited to email addresses, telephone, or mobile numbers, will not be shared with third parties/affiliates for marketing/promotional purposes.

We also may use the information we obtain about you in other ways for which we provide specific notice at the time of collection.

Additionally, we will share personal information with third-party companies, organizations or individuals outside of Company if we have a good-faith belief that access, use, preservation or disclosure of the information is reasonably necessary to:

Transfer in the Event of Sale or Change of Control 

If the ownership of all or substantially all of our business changes or we otherwise transfer assets relating to our business or the Site to a third party, such as by merger, acquisition, bankruptcy proceeding or otherwise, we may transfer or sell your personal information to the new owner. Should such a sale, merger, or transfer occur, we will use reasonable efforts to direct the transferee to use your personal information in a manner that is consistent with our Privacy Policy.

Automatically Collected Information

When you visit the Site, some information is automatically collected from you. This information may include, but is not limited to, some or all of the following items: your Internet Protocol (IP) address and/or domain; type and version of Internet browser software and operating system you use; your general location; date, time, and duration of your Site access; specific pages, buttons, images, videos, links, or forms that you access while visiting the Site; type of device (e.g. iPad, smartphone); mobile carrier and/or Internet Service Provider; and demographic information concerning the country of origin of your device and the language(s) used by it.

Information Collected via Cookies

Cookies are small text files that are placed on your device to distinguish you from other visitors to our Site. The use of cookies is a standard practice among websites to collect or track information about your activities while using the website. We and/or our third-party advertising service providers may place cookies or similar files on your hard drive for many of the reasons listed in this Privacy Policy while visiting our Site. Information Collected via Flash Objects and Other Similar Technology

We may partner with third parties such as Google to deliver advertisements and monitor activities on our own website and other websites. We do not, however, track your online activities after you leave our Site. “Do Not Track” Signals.

You can access and update certain information we have relating to your ClearBalance account by signing into your account at MyClearBalance.com and going to the account section of the Site. Security

No Information from Children Under the Age of 13

If you are under the age of 13, please do not attempt to register with us at this Site or provide any personal information about yourself to us. The Site is not directed to, and we do not knowingly collect or solicit personal information from, children under the age of 13. If we learn that we have collected personal information from a child under the age of 13, we will promptly delete that information. If you provide your email address to us, or if we obtain it from another source, we may use it to respond or update you on your inquiries or to let you know about products and services that may be available to you. If your name was previously selected for any of our upcoming email offers, you may receive information from ClearBalance up to 15 business days after your request has been submitted.

California Civil Code Section § 1798.83 permits users of the Site who are California residents to request certain information regarding our disclosure of personal information to third parties for their direct marketing purposes. Our Site is operated and managed on servers located and operated within the United States.
